Join our Emergency Department team at Flinders Medical Centre to make a difference in the Older Persons Service!

The Single Point of Contact' Nurse Consultant provides support to the Emergency Department (ED) Team in the management of older people presenting to the ED who have been identified as having complex psychosocial, functional and acute medical needs. They provide advice and care coordination for older people presenting to (or at risk of presenting to) ED, with a particular focus on frail older people and those living in Residential Aged Care Facilities.

About the Role:

Reporting to the Nursing Director of Emergency Department, the Nurse Consultant is responsible for the coordination, monitoring and evaluation of the Older Person Service strategy which includes:

- Providing a single point of contact for Residential Aged Care Facilities and GPs to support decision making regarding the need for an older person to present to ED.
- Assisting with decisions regarding eligibility for admission under the Older Persons Assessment Liaison (OPAL) unit and Residential Care Outreach Team (RCOT)
- Delivering expert clinical consultation and assessment for older persons across inpatient and relevant community-facing settings
- Driving evidence-based practice improvements in aged care and frailty management
- Supporting and mentoring nursing staff to build capability in older persons care
- Collaborating within multidisciplinary teams to optimise patient outcomes and care transitions

About you:





- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Specialist level of knowledge, clinical assessment and practice skills required for the care of the Older Person in an ED environment
- Demonstrated experience in working with vulnerable older adults either acutely or within a facility setting.
- Demonstrated experience in older persons or complex care settings within an acute or tertiary environment
- Proven ability to lead clinical practice change and translate evidence into bedside outcomes
- Strong communication and interprofessional collaboration skills across diverse clinical teams
