You will work alongside a dedicated multidisciplinary team supporting people experiencing complex health and substance use challenges in the community.Be part of a specialised Drug Health service supporting vulnerable and diverse communitiesWork collaboratively with nurses, clinicians and other health professionalsContribute to a culture of respect, open communication and shared problem solvingSupport people and their carers through compassionate, culturally appropriate careHelp drive continuous improvement and evidence-based practice across the serviceWhat you'll be doingAs a Registered Nurse, you will provide safe, person-centred and evidence-informed nursing care while delivering assertive community-based Drug & Alcohol case management.Assess, plan, implement and evaluate individualised nursing careProvide comprehensive case management to people living in the communityBuild therapeutic relationships with patients, carers and the multidisciplinary teamMaintain accurate clinical documentation, care plans,
