Working under the guidance of experienced Care Managers, you will build practical capability across client coordination, care planning, documentation and stakeholder communication.Reporting to the Business Operations Manager, this developmental role is suited to someone transitioning from a Caregiver, Care Administrator or similar position while working towards a Certificate IV in Ageing Support, or an equivalent qualification.

follow-up and person-centred supportAssist with coordinating care services for Private clients, including onboarding, service commencement and selected client visitsSupport client reviews, goal discussions and simple care plan updates as your capability developsMaintain accurate and timely client records, document stakeholder interactions and support quality and audit-ready documentationMonitor caregiver feedback and promptly escalates risks, incidents or concerns to the supervising Care ManagerCollaborate with Care Managers, Care Administrators and the wider office team while actively participating in training, case discussions and coachingAbout youYou are a caring, organised and relationship-focused professional who is ready to take the next step in your career.
