HACC Worker / Community Care Worker Duties Provide practical support to elderly people and people with disability. Assist clients with daily living activities and personal care, where required.

Support clients with meal preparation, shopping and household tasks. Provide transport or accompany clients to medical appointments and community activities. Encourage clients to remain independent, active and connected with the community.

Provide companionship and social support. Monitor clients’ wellbeing and report any concerns or changes to the supervisor. Follow individual care plans and workplace procedures.

Maintain accurate client notes and service records. Respect clients’ privacy, dignity, culture and personal choices. Follow infection-control, manual-handling and workplace health and safety requirements.

Communicate professionally with clients, families, health services and other support workers. NO ACCOMMODATION PROVIDED
