Job Description

Sincera Supports is seeking Support Workers to join our team in Perth. As a Support Worker, you will work closely with individuals living with neurological diseases, such as MND, MS, Stroke, SCI, and ABI, to provide them with compassionate care and support. Your role will involve assisting with daily activities, ensuring a safe and healthy home setting, and helping participants with community activities and outings.

Responsibilities
Develop solid and meaningful relationships with participants and their families
Provide personal care, support, and supervision to participants
Create a safe and healthy home workplace for participants
Assist participants with community activities and outings
Follow individual care plans and goals to meet participants' specific needs

Requirements
Proven experience in providing personal care within a work setting
Certificate III in Individual Support/Aged Care & Community/Disability
First Aid certificate with current CPR
Manual Handling certificate
NDIS Workers Screening Check
NDIS Workers Orientation Module
Working with Children Check or ability to obtain one
Medication Competent
Willingness to be PEG-competent
