Life Without Barriers invites applications for a Case Manager (Family Based Care) based in Parramatta.
The role is permanent full time with hybrid arrangements, working Monday–Friday, 8:30am–5pm.
You will support young people in out-of-home care, applying trauma-informed case management and building life skills.
As part of the role you will conduct monthly home visits across the Northern and Western Sydney regions.
Probity checks are required, and some travel may be needed depending on caseload.
