Provide a central point of contact for screening and assessment of young people and families referred to the service
Coordinate service provision and help clients access appropriate support
Collaborate to obtain organisational information regarding clients, individual and family needs, current service delivery and service gaps
Build and maintain relationships with government, non-government, community stakeholders and local service providers
Coordinate and participate in cross-agency case discussions, local governance groups and network meetings
Develop partnerships with service providers, understand available services, develop referral pathways and negotiate services
Advocate regarding access and service delivery
Identify options for brokering external service provision where supports are unavailable
Monitor program activities, deliverables and outcomes
Identify risks, emerging issues and opportunities for continuous improvement
Apply ethical and evidence-informed practice, including social justice, empowerment, cultural safety and child-centred approaches
Support meaningful participation of young people and families in decision-making
Requirements
Demonstrated understanding and experience of the service system which supports children and their families, and the interconnectivity between government and non-government agencies involved in supporting families
Strong communication skills
Ability to establish and maintain relationships with government and non-government agencies




Confident communicator able to engage and support a diverse range of stakeholders
Strong engagement and relationship building skills
Highly resilient and able to work under pressure and manage competing priorities
Ability to understand and adhere to Legislation, Policies and Procedures
Ability to plan and prioritise workload with attention to detail
Current driver's licence and willingness to travel
Subject to a National Criminal History Record Check
Subject to a Working with Children Check clearance
