About the Role
Our client is a registered organization dedicated to community development and support services. We are seeking a dedicated Community Worker to join their team and deliver impactful programs across local communities.
Key Duties and Responsibilities
Conduct in-depth client assessments to identify needs, then design and implement tailored educational, training, and support programs that address social, emotional, and financial challenges.
Interview clients to evaluate the nature and extent of their difficulties, and develop individualized case plans with clear, measurable goals.
Monitor client progress through regular check-ins, document outcomes, and prepare detailed reports for internal review and external agencies.
Refer clients to appropriate services—such as housing, employment, or health providers—and coordinate multi-agency support to ensure holistic care.
Assess community needs and resources related to welfare, housing, employment, training, and other facilities, then advocate for improved services.
Liaise with community groups, welfare agencies, government bodies,



and private businesses to address local issues and promote awareness of available resources.
Support families and care for children, disabled persons, or elderly clients in group housing, adult service units, and institutional settings.
Supervise offenders on probation or parole, ensuring compliance with court orders and facilitating rehabilitation programs.
Assist young people in resolving social, emotional, and financial difficulties through counselling and practical guidance.
Prepare funding submissions and detailed reports for government bodies and other agencies to secure resources for community initiatives.
Requirements
Minimum 1 year full time relevant experience (disability support, community services, or allied health assistance).
Relevant qualification (e.g., Certificate IV/Diploma in Community Services, Disability, or Mental Health).
Strong interpersonal and communication skills for effective liaison with diverse stakeholders.
Ability to work independently and collaboratively within a multidisciplinary team.
Current driver's license and willingness to travel within the community.
