The role focuses on culturally secure, person-centred care, delivering health assessments, education, and care coordination for Aboriginal communities across Adamstown and Raymond Terrace.
You will work across homes, clinics, and telehealth, using clinical expertise and cultural knowledge to connect clients with the right health services and support chronic disease
