Working with clients, families and service providers, you'll coordinate quality care and lead a team focused on positive outcomes across the community.Every day, you'll contribute by:Coordinating personalised support that empowers clients to achieve their goals and make informed choicesBuilding strong partnerships with carers, health professionals and community services to deliver seamless careLeading and supporting home care staff to maintain secure, responsive and high-quality servicesDriving service improvements, compliance and resource management that strengthen client outcomesLet's talk about you!You're someone who combines compassion with sound judgement and enjoys helping people navigate important life stages with confidence. You build trusted relationships, adapt to changing needs and take pride in creating positive experiences for older people and their families. The difference you make is seeing clients supported to live well at home while receiving care that reflects their goals, dignity and independence.Here's what will help you thrive in this role:Experience coordinating community-based or aged care services, including care planning and case managementStrong communication skills and the ability to work collaboratively with clients, carers and multidisciplinary teamsExperience leading staff and managing priorities in a dynamic service environmentConfidence using digital systems, databases and administrative processes to support quality careYou'll stand out from the crowd if you have a Certificate III or higher in Individual Support (Ageing) or similar and/or experience working with aged care quality standards.Above all, you'll bring compassion, integrity, dignity, innovation and courage to every interaction,
