Bring your skills to a unique opportunity with APM as a Clinical Aged Care Assessor, where you will play a pivotal role in identifying the needs of older adults and connecting them with services that support independent living and the highest possible quality of life.
n
As the role evolves, you will also step into supporting a broader team across the region. You'll enjoy a high level of autonomy, travelling to aged care homes to complete Residential Aged Care Funding Assessments using the AN-ACC tool, ensuring accurate, person-centred outcomes for residents and providers.

What you bring:
n
n
5 years' experience post graduate experience in aged care including working with people with dementia.
n
Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
n
Ability to conduct clinical assessments using a range of assessment tools.
n
Strong organisational skills and the ability to work independently and as part of a team.
n
A genuine desire to improve the lives of older Australians.
n
Your own vehicle as travel will be required within your region.
