What you will do
Provide comprehensive physiotherapy assessments and treatments for adults within community settings.
Coordinate and deliver rehabilitation interventions tailored to adults with complex needs.
Engage with patients in their own homes, residential facilities, and community rehabilitation centres.
Collaborate effectively with carers, GPs, Social Services, nursing teams and voluntary organisations to optimise patient care.
Maintain meticulous clinical records and prepare detailed reports, ensuring continuity of care.
Operate autonomously as a practitioner, adapting to versatile environments.
Oversee the delegation and supervision of rehabilitation assistants as necessary.
Participate actively in service development, audits, and governance to enhance the quality of service.
Contribute to multidisciplinary rehabilitation planning, fostering cohesive team dynamics.
Requirements
Possess a degree in Physiotherapy and be registered with the HCPC.
Bring experience from within the NHS, specifically in Community and Domiciliary Physiotherapy.
Demonstrate the ability to function autonomously and effectively within community settings.
Possess solid communication and documentation skills, essential for multi-disciplinary collaboration.
Experience in community rehabilitation teams and neurology would be advantageous.
Capability to supervise and teach assistants or students is desirable.
