About the role

The Social and Emotional Wellbeing (SEWB) Case Manager provides case management to clients referred for SEWB support, with a particular focus on mental health. This role involves coordinating and delivering individualised support, which may include advocacy, outreach, skill development, life skills training, facilitating access to community and Congress services and coordinating interagency meetings.

Clients often experience multiple and complex needs, including co-occurring mental and physical health concerns, family and community violence, alcohol and other drug (AOD) issues, child protection involvement, engagement with the justice system, cultural considerations, and other social and emotional wellbeing challenges.

To be considered for this position, you will require:

- Is of Aboriginal descent; identifies as an Aboriginal person; and is accepted as an Aboriginal person by the Aboriginal community
- Demonstrated experience and skills in providing case management to vulnerable clients that is trauma-informed and strengths based.
- Demonstrated knowledge and understanding of Social and Emotional Wellbeing (SEWB) principles and practice.
- Demonstrated ability to foster and maintain team-oriented working partnerships with internal and external providers.
- Understand and maintain strict confidentiality in a health care setting as well as cultural sensitivity within the Aboriginal community.




- Awareness of and sensitivity to Aboriginal culture and history and knowledge of factors influencing the health and wellbeing of Aboriginal children and families in Central Australia.
- Current NT Driver's License or the ability to obtain.
- Willingness, ability and appropriate levels of health and fitness to travel to and work from remote locations.
