Aged Care On-site Pharmacist

Positions available throughout the Sunshine Coast, Brisbane, Gold Coast and Northern NSW

Chempro Aged Care Chemists is seeking full-time or part-time Aged Care On-site Pharmacists (ACOP) to join our dedicated aged care team. These resident-facing clinical roles are based within partner residential aged care homes and focus on improving medication safety and the appropriate use of medicines. The role requires pharmacists with ACOP accreditation. Current accreditation preferred but we can also support you in your accreditation process.

You will work closely with residents, families, nurses, general practitioners, allied health professionals and our experienced pharmacy team. The role suits a pharmacist who wants to use their clinical knowledge in a team-oriented environment and build trusted relationships with residents and care teams.

Key responsibilities

• Identify and resolve medication-related problems, including risks associated with polypharmacy and high-risk medicines.

• Support medication reconciliation following admissions, hospital transfers and changes in therapy.

• Provide clinical advice to residents, families, nursing staff, prescribers and other health professionals.

• Contribute to medication reviews, case conferences, clinical governance meetings and quality use of medicines activities.

• Review medication incidents and audit findings, then assist the home to implement practical improvements.

• Deliver medication education and support to residents and aged care staff.





• Document interventions clearly and communicate recommendations to the wider care team.

About you

• Bachelor of Pharmacy or equivalent qualification with current AHPRA registration.

• Strong clinical knowledge and a genuine interest in improving care for older people.

• Excellent communication and relationship-building skills.

• Confidence working autonomously while contributing to a multidisciplinary team.

• Sound judgement, initiative, organisation and computer skills.

• Aged care, hospital or other clinical pharmacy experience is desirable but not essential.

• Medication review credentialing would be highly regarded but is not essential.

• A current driver's licence and the ability to travel locally within your nominated region.
