We are currently seeking passionate Registered Nurses or Enrolled Nurses to join our growing team as a Chronic Disease Management Health & Wellbeing Nurse.
This is an exciting opportunity to play a key role in preventative healthcare and chronic disease management, working closely with GPs, allied health professionals, and patients to improve long-term health outcomes through education, care coordination, and ongoing support.

About the Role
In this role, you will:

Support patients to manage chronic conditions and achieve positive health outcomes
Provide health coaching and promote positive behaviour change initiatives
Coordinate care with GPs, specialists, allied health providers and community services
Conduct and coordinate clinical services including ECGs, Spirometry, ABI testing and DMMRs where appropriate
Maintain accurate patient records and ensure Medicare compliance requirements are met
Proactively identify eligible patients for chronic disease management and preventative health services
Build strong relationships with patients and provide ongoing education and support

About You
To be successful in this role, you will have:





Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EN)
Previous experience in Chronic Disease Management, Care Plans and Health Assessments (highly regarded)
Prior experience in General Practice or Primary Healthcare (preferred)
Strong understanding of preventative health, health promotion and chronic disease management principles
Excellent communication and interpersonal skills
Strong organisational and time management abilities
The ability to motivate and engage patients in improving their health outcomes
A proactive, patient-focused approach to care
