- Recommend equipment and home modifications
- Deliver falls prevention strategies

- Deliver coordinated care alongside Clinical Care Partners, nurses and allied health professionals
- Contribute to the growth and development of the Support at Home service
- Support prevention, early intervention and reablement for older Australians

Requirements

- Current unrestricted AHPRA registration as an Occupational Therapist
- Ability to work autonomously and collaboratively within multidisciplinary teams
- Excellent organisational and time management skills
- Current driver licence
- Access to own vehicle
- Willingness to travel
