- Provide assessment, brief intervention, case management and coordinated care to adults with complex mental health needs
- Conduct comprehensive mental health assessments and develop individualised, recovery-oriented treatment plans
- Deliver evidence-based psychological and psychosocial interventions
- Provide case management and care coordination across participants’ care journeys
- Partner with GPs and healthcare providers to deliver shared care, clinical consultation and coordinated support
- Facilitate referrals to appropriate health, community and support services
- Collaborate with a multidisciplinary team to provide integrated, person-centred, trauma-informed and culturally safe care
- Maintain accurate clinical records, manage risk and safety planning, and ensure compliance with clinical governance and quality standards
Requirements
- Qualified in Social Work, Psychology, Occupational Therapy, Counselling, Mental Health Nursing or a related discipline
- Experience supporting people with complex mental health needs through assessment, intervention and case management
- Skills in risk assessment, safety planning and evidence-based, recovery-focused practice
- Comfortable working independently and as part of a multidisciplinary team
- Ability to build relationships with clients, families, GPs and community partners
- Strong communication, time management and clinical documentation skills
- Commitment to person-centred, trauma-informed and culturally secure care
- Proof of COVID-19 vaccination status, or as required by legislation and WMQ policy
- Relevant probity checks required by legislation and WMQ Limited policy
- Current driver’s licence and willingness to travel for work
