The Salvation Army Australia’s Homelessness Stream delivers high-quality, person-centred services to individuals and families at risk of or experiencing homelessness. We provide accommodation, case management, outreach, financial assistance, and connections to specialist services – supporting people to rebuild independence and stability.

About the role

Join Foster House and make a meaningful difference in the lives of individuals and families experiencing homelessness. As a Case Manager, you will provide trauma-informed, person-centred support that empowers clients to achieve housing stability, strengthen social connections, and build pathways to independent living. Reporting to the Team Leader, this is a full-time permanent role based in Surry Hills. Salary is in accordance with SCHADS Crisis Level 2.

How you will make an impact

- Deliver case management, crisis intervention, and transitional support in line with the National Homelessness Stream Model of Care.
- Develop, implement, and regularly review tailored case and care plans with clients.
- Support residents and outreach clients through ongoing case management and advocacy.
- Conduct risk assessments and safety planning for outreach and offsite activities.
- Maintain accurate client records, case notes, and reporting requirements.
- Collaborate with multidisciplinary teams and external agencies to achieve positive client outcomes.
- Contribute to a safe, inclusive, and culturally responsive service environment.

What you will bring





- Diploma or higher qualification in Community Services, Social Work, or a related field (or currently working towards a degree).
- Experience in case management and supporting people experiencing homelessness, mental health challenges, AOD issues, or other complex needs.
- Sound understanding of trauma-informed practice and client-centred approaches.
- Strong stakeholder engagement, communication, and documentation skills.
- Ability to work collaboratively and adapt to changing service needs.

Eligibility & background checks

- NSW Working with Children Check.
- Willingness to complete a Nationally Coordinated Criminal History Check.
- Current Driver’s Licence (desirable).
- Flexibility to travel and work varied hours as required.
