We're seeking experienced and empathetic professionals to deliver high-quality, person-centred support needs assessments for people living with disability.

In this role, you will gather and analyse information, interpret evidence, and prepare transparent, accurate assessment reports to support informed decision-making. You'll communicate respectfully with participants, families and support networks, using trauma-informed and culturally-safe approaches to ensure people feel heard, understood and supported.

Key Responsibilities will include:

- Conduct support needs assessments through guided conversations and information gathering.
n
- Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
n
- Prepare clear, accurate and defensible written assessment reports.
n
- Apply policy, legislation and procedures using sound judgement and ethical practice.
n
- Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
n
- Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
n
- Collaborate across teams to support coordinated and inclusive participant outcomes.
n
- Maintain accurate records using digital systems while ensuring privacy and confidentiality.

About You
n
You are a compassionate and adaptable professional with strong assessment, interviewing and stakeholder engagement skills. You can confidently manage complex information, apply sound judgement, and communicate effectively with diverse audiences.

You will demonstrate:

- Experience conducting support needs assessments or similar person-centred assessments.

- Strong analytical, report writing and decision-making capability.
n
- Commitment to inclusive, accessible and culturally-safe practice.

- Trauma-informed and strengths-based approaches to participant engagement.

n
