What you will do

n

n
- Manage a varied caseload of adults receiving support from secondary mental health services, delivering person-centred interventions and ongoing case management.
n
- Complete strengths-based assessments, develop care and support plans,



and regularly review outcomes to promote independence and recovery.
n
- Work closely with the Transition Team, supporting young people moving into adult mental health services. Experience within Children's Social Care is desirable but not essential.
n
- Collaborate with service users, families, healthcare professionals, and partner agencies to provide coordinated, holistic care.
n
- Maintain accurate, timely, and legally compliant case records and documentation in line with statutory requirements and professional standards.

Requirements

n

n
- Qualified Social Worker with current Social Work England registration.
n
- Experience working within Adult Social Care, ideally in a Community Mental Health Team or similar setting.
n
- Strong understanding of the Care Act 2014, Mental Health Act, Mental Capacity Act, and related legislation.
n
- Excellent assessment, care planning, communication, and multidisciplinary working skills.
n
- Ability to manage a busy caseload while delivering high-quality, person-centred practice.
