Illawarra Shoalhaven Local Health District is seeking a Social Worker to join the Community Chronic Care Service in Wollongong on a permanent part time basis (14 hours per week).

You will provide specialist social work services, clinical leadership and supervision to Level 1-2 social work staff and students, delivering care via telehealth, virtual and face-to-face sessions to patients with life-limiting chronic illnesses.
