Job Description

Southern NSW Local Health District is seeking a Social Worker Level 3 in the Out of Hospital Care Community Chronic Care Program at Wollongong Community Health Centre. The role provides specialist social work services, supervision of Level 1-2 staff and student supervision, and contributes to discharge planning and telehealth care.
