About the Role
n
As a Physiotherapist, you will play a key role in delivering high-quality, evidence-based care to older adults living in the community, with the opportunity to see the direct impact of your work every day.
n
Working within a supportive multidisciplinary team and reporting to the Clinical Manager, you will help clients maintain their mobility, independence, function and safety through tailored assessments, goal-focused interventions, exercise programs and rehabilitation.
n
Your work will enable older people to remain living confidently in their own homes, maintain their physical wellbeing and continue engaging in the activities that matter to them.
n
You will work closely with clients, families, Case Managers, and allied health professionals to deliver coordinated, person-centred care aligned with individual goals and preferences. This role requires regular travel across the community to visit clients in their homes.

This is a rewarding opportunity to join a supportive and collaborative team as a full-time Physiotherapist, based in either Devonport, Launceston, or Hobart. We are seeking a compassionate and motivated clinician with a strong client-centred approach, excellent communication skills, and a genuine passion for supporting older adults.

As a Physiotherapist, you will:
n
n
- Conduct comprehensive physiotherapy assessments and maintain accurate, compliant documentation
n
- Develop, implement, and review individualised physiotherapy treatment plans to support client goals and outcomes
n
- Assess and manage mobility, strength, balance, pain, physical function and falls risk
n
- Develop and prescribe individualised exercise, strengthening, balance and rehabilitation programs
n
- Provide evidence-based physiotherapy interventions to improve or maintain mobility, independence, function and quality of life
n
- Assess and recommend appropriate mobility aids and equipment where required
n
- Monitor client progress and respond to changes, escalating concerns as needed
n
- Identify and address falls risks and implement appropriate prevention strategies
n
- Coordinate referrals and collaborate with multidisciplinary and external providers
n
- Provide education and practical support to clients,
carers, and support staff
n
- Contribute to care planning, case conferences, and client-centred service delivery
n
- Support risk management, compliance, and continuous improvement activities
n
n
About You
n
Essential
n
n
- Current AHPRA registration as a Physiotherapist
n
- Strong clinical reasoning and assessment skills, with a commitment to evidence-based, person-centred care
n
- Experience in community, aged care, or rehabilitation settings, supporting independence and wellbeing
n
- Excellent communication skills, with the ability to build rapport and collaborate across multidisciplinary teams
n
- Demonstrated ability to manage a caseload, prioritise effectively, and make sound clinical decisions
n
- Strong knowledge of physiotherapy assessment and treatment, including mobility, exercise, rehabilitation and falls prevention
n
- Solid understanding of risk management, including secure work practices in home and community settings
n
- Strong organisational, problem-solving, and time management skills in a dynamic environment
n
- Current National Police Check and unrestricted driver's licence
n
n
Desirable
n
n
- 2-3 years' experience as a Physiotherapist within home care, community, or aged care settings
n
- Demonstrated experience working with older adults and supporting mobility, strength, balance and independence
n
- Demonstrated experience in falls prevention, exercise prescription and rehabilitation
n
- Experience in mobility assessment and prescription of appropriate mobility aids and equipment
n
- Sound understanding of Aged Care Quality Standards, NDIS Practice Standards, and relevant legislation
n
- Knowledge of wellness, reablement, and restorative care approaches
n
- Experience in quality improvement activities, or a willingness to develop capability in this area
