Your new role:

As a Social Worker at Logan Hospital, you'll provide high-quality social work services across inpatient and outpatient settings, supporting patients, families and significant others through complex health and social circumstances. You'll undertake clinical assessment, therapeutic intervention, patient education and discharge planning while working collaboratively within an interprofessional team. You'll also have opportunities to contribute to student education, service improvement and research while continuing to develop your clinical expertise.

Key responsibilities

- Deliver evidence-based assessment, intervention and evaluation across inpatient and outpatient settings.
- Support patients and families through counselling, education, advocacy, referrals and discharge planning.
- Collaborate with multidisciplinary teams to achieve safe, sustainable and patient-centred outcomes.
- Contribute to student education, quality improvement, research and service development initiatives.

About you:

You are a compassionate and capable Social Worker who can build strong relationships with patients, families and healthcare professionals. You bring sound clinical judgement, effective communication and a commitment to evidence-based practice, while being motivated to continue developing your skills within a supportive multidisciplinary environment.

Skills & experience

- Tertiary qualification in Social Work and eligibility for AASW membership.
- Demonstrated ability to provide high-quality, patient-centred social work services.




- Strong communication and teamwork skills within an interprofessional environment.
- Commitment to professional development, clinical education and service improvement.
