About the Role
We're looking for a passionate and dedicated Occupational Therapist Grade 2 to join our team part-time ongoing at Monash Health in a 0.8EFT position.
You will be working within the geriatric evaluation and management (GEM) stream to provide meaningful intervention to older frail adults.
You'll be part of a large, supportive subacute service, delivering high-quality occupational therapy in a collaborative, multidisciplinary environment.
This is an exciting time to work in GEM with several model of care changes that look at making a real impact to the frail older population.
This role is fast-paced, rewarding, and integral to delivering exceptional rehabilitation outcomes.
As an Occupational Therapist, you'll empower patients to engage in meaningful activities and improve their quality of life.
Your day will include:
Conducting occupational assessments
Collaboratively setting goals with patients and the multidisciplinary team
Delivering occupation-based interventions that have a real impact




You'll work closely with allied health, nursing, and medical staff in a supportive setting that encourages shared learning and joint treatment planning.
You will have supervision and support to have hands on clinical learning with:
Grade 3 occupational therapists
Clinical educators
Clinical grade 4 specialists
About you
You're a compassionate and skilled clinician who thrives in a team environment and is committed to delivering exceptional care.
You bring:
A patient-centred approach to care
Proven ability to work collaboratively in a multidisciplinary team
Experience in subacute clinical settings and experience with a neurological caseload desired
A passion for enhancing occupational performance and patient experience
A commitment to ongoing learning and professional growth
Strong communication and interpersonal skills
Interest in evidence-based practice, research, and service improvement
