Dhelkaya Health is seeking a skilled and compassionate At Home Nurse to join our dynamic At Home Program, delivering high-quality clinical care to clients in the comfort of their own homes.

Our At Home Program includes:
n
n
Hospital in the Home (HITH)
n
Geriatric Evaluation Management (GEM) at Home
n
Residential In Reach (RIR)
n
Rehabilitation in the Home (RITH)
n
n
Operating 7 days a week, the program provides a safe and effective alternative to hospital-based care through coordinated, multidisciplinary service delivery.
n
Key Responsibilities
n
n
Deliver expert, client-centred nursing care in home settings
n
Develop and implement individualised care plans
n
Advocate for clients and support their health goals
n
Provide education to clients and carers to promote independence and wellbeing
n
Monitor and respond to clinical deterioration and changing needs
n
Collaborate with multidisciplinary teams to ensure coordinated, high-quality care
n
Support transitions of care, including discharge planning and referrals to other services
n
You will be actively involved across all phases of care, ensuring clients receive the right level of support at every stage.

Demonstrates strong clinical assessment and decision-making skills

Has experience or interest in community, aged care, or acute care services
n
Is passionate about delivering patient-centred care in a home environment

Communicates effectively with clients, families, and multidisciplinary teams

Is adaptable, organised, and able to work independently
