Transition Care Program – Home Based is a jointly funded program (State/Commonwealth) and aims to assist frail aged patients to complete their restorative processes at home, optimise their functional capacity and finalise their long-term care arrangements.
The Team leader will take a leading role within the Transition Care Program – Home Based, including quality planning, policy review, budget management, aged care compliance and ensuring the services are aligned with organisational objectives and professional standards.
The role is responsible for identifying and resolving client and staff issues, while overseeing service delivery to ensure a high-quality experience for clients.
Working closely with the Program Clinical Lead, you'll support effective client service provision, coordination and flow across the program.
Key Accountabilities
Work closely with a highly experienced multidisciplinary team to support the delivery of case management to older people transitioning from hospital to home.
Work collaboratively with internal and external stakeholders to build strong relationships and partnerships, strengthen service models and improve patient flow.
Provide leadership and support to the team, contributing to quality service delivery, continuous improvement and effective resolution of client and staff issues.
Contribute to the ongoing development of the team and service, supporting initiatives that enhance care for older people and strengthen Community Services across RMH.
Support quality planning, compliance and resource management, ensuring the program aligns with relevant aged care requirements, organisational objectives and professional standards.




Requirements
Registration as a Division 1 Registered Nurse or relevant experience as an Allied Health Professional.
A strong understanding of aged care services and legislative requirements, including My Aged Care.
Experience working with clients, families, support networks and service providers to monitor and evaluate service delivery and care needs.
Demonstrated skills in comprehensive assessment and discharge planning.
Sound knowledge of hospital processes, aged care and community service systems.
Experience supporting older people, including those with complex social circumstances and care needs.
Experience working across hospital, aged care and/or community settings.
Previous experience leading and supervising teams, with the ability to support staff and promote effective service delivery.
A versatile, proactive and innovative approach, with the ability to work effectively both autonomously and collaboratively within a multidisciplinary team.
