The Opportunity We are looking for an experienced and passionate leader to join our Brightwater At Home team as Service Lead - Transition Care Community (TCC) .
This is a rewarding leadership opportunity where you'll play a pivotal role in supporting safe, timely and person-centred transitions from hospital to home, while leading a high-performing team of Transition Care Coordinators.

What you'll bring A relevant tertiary qualification in Nursing, Allied Health, Health Services, Community Services or a related discipline Leadership and operational management experience within health, aged care or community services Experience managing referrals, admissions, care coordination, discharge planning or transition services Robust people leadership skills with demonstrated success developing and supporting teams Excellent stakeholder engagement and relationship management capabilities Experience monitoring service performance, data and financial outcomes High-level communication, problem-solving and decision-making skills Strong digital literacy across Microsoft Office and client management systems A satisfactory National Police Clearance issued within the past 12 months.
A current valid WA driver's licence.
