The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services and eligible aged care clients.
About the Role?
Your day-to-day duties will include: Demonstrate practical and innovative leadership in the design implementation and evaluation of daily programs for clients.
Liaise with the Assessor and Community Nurses to ascertain Client needs.
Identify a range of services that enhance inclusion for elderly Indigenous persons who are residing in their own homes within the community.
Collaborate with other service providers to ensure effective service delivery and increase community networks.
Coordinate and review service arrangements with Service Providers.
About You?
Current Criminal History Check.
Current C Class Drivers Licence (QLD).
Current Disability Worker Screening Check (Qld Yellow Card).
Current First Aid and CPR Certificates.
Desirable, but not mandatory: Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
