This role leads the hospital avoidance team and focuses on cutting-edge patient centred care within a hub and spoke model, working collaboratively with local referrers and community services.

In this role, you will have the opportunity to:
Collaborate with multidisciplinary teams to enhance patient outcomes and health servic e quality
Develop and implement innovative healthcare strategies that align with best practices and organisational goals
Develop your Scope of Practice to improve patient centred health care
D emonstrated Advanced Clinical Practice in the main Metaspecialty, focusing on complex, autonomous decision-making in the clinical management of patients.
Demonstrated willingness in the development of your Scope of Practice to improve access to healthcare for patients and their carers.
Demonstrated effective communication and teamwork skills, building strong relationships while maintaining professional boundaries.
Demonstrated leadership and educational activities in your current role.
Demonstrated ability to support health care systems and research.
