About the role
n
The Clinical Project Lead - Dementia Model of Care will lead the implementation and evaluation of Calvary's Dementia Model of Care across hospital, residential aged care and home care services.
n
Working collaboratively with consumers, families, carers, clinicians and operational leaders, the role will support the delivery of consistent, person-centred and evidence-informed dementia care across Calvary services. The position will lead project activities, stakeholder engagement, education and capability-building initiatives to embed the model of care and improve outcomes and experiences for people living with dementia and their families.

The role will partner closely with service leaders and frontline teams to assess readiness, monitor implementation, address barriers, and drive continuous improvement and sustainable practice change.
n
Your main duties will include:
n
n
Lead implementation and continuous improvement of the Dementia Model of Care across designated Calvary services and care settings, ensuring consistent and sustainable adoption.
n
Lead consultation and co-design with people living with dementia, families, carers, staff and multidisciplinary teams, ensuring lived experience informs implementation and evaluation.
n
Develop and manage implementation plans and governance,
including scope, milestones, responsibilities, dependencies, stakeholder engagement, reporting, and escalation of risks and issues.
n
Assess service readiness and support local implementation, working with operational leaders and teams to identify barriers, enablers, priorities and opportunities for local adaptation.
n
Build capability and embed evidence-based practice by supporting implementation champions and developing education, resources, coaching and practical tools aligned with the model of care.
n
Monitor, evaluate and drive improvement, using implementation data, quality indicators and stakeholder feedback to measure impact, identify variation, inform recommendations and support the long-term sustainability of the model.

About you
n
Essential
n
n
Relevant tertiary qualification in a health discipline (e.g. nursing, medicine, allied health).
n
Experience coordinating and delivering healthcare or aged care improvement projects.
n
Demonstrated understanding of dementia care and person-centred practice.
n
Experience engaging with a broad range of stakeholders, including clinicians, operational leaders, consumers and families.
n
Strong organisational and project coordination skills,
including management of timelines, work plans and deliverables.
n
Highly developed communication, influencing and stakeholder engagement skills.
n
Values-driven leadership with a robust commitment to dignity, compassion and person-centred care
n
n
Desirable
n
n
Experience working across integrated care systems.
n
Experience leading change in large, complex healthcare organisations.
n
Understanding of digital health systems and data-driven improvement.
n
