Nepean Blue Mountains Local Health District is seeking a Mental Health Intake and Referral Clinician who will review referrals via the Patient Flow Portal and coordinate access to community mental health services for consumers.

You will conduct mental health triage assessments, coordinate warm handovers to receiving teams, and provide post-discharge follow-up support as patients transition from inpatient care to the community.
