Our Clinical Care Coordination services provide intensive, wrap-around supports for people experiencing severe and persistent mental health challenges, bringing together clinical care, psychosocial support, and access to essential community services.
n

n
We're seeking a compassionate and skilled Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.
n
Working closely with a multidisciplinary team, you'll independently support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, cooperative practice.

Responsibilities
n
n
Conducting clinical assessments and developing tailored treatment plans
n
Deliver evidence-based psychosocial interventions and service improvement
n
Autonomously manage caseload and conducting risk assessment with management plans
n
Collect data and supporting monitoring and evaluation
n
Collaborate with teams and external stakeholders
n
Facilitate care planning and review processes
n
Monitor and elevate mental health needs as appropriate
n
Access to salary package up to $15,900 and $2,650 for meal and entertainment p/a of tax-free income

Requirements
n
n
Mental Health Accreditation
n
Current AHPRA practicing certificate or AASW registration
n
Tertiary qualification in psychology, social work, mental health nursing, or similar
n
Experience managing complex mental health cases and trauma-informed care
n
Experience working with diverse cultures, especially First Nations people
n
Strong teamwork and collaboration skills, problem-solving and organisational abilities
n
