, where you will play a pivotal role in identifying the needs of older adults and connecting them with services that support independent living and the highest possible quality of life.
As the role evolves, you will also step into supporting a broader team across the region. You'll enjoy a high level of autonomy, travelling to aged care homes to complete
Residential Aged Care Funding Assessments
using the
AN-ACC
tool, ensuring accurate, person-centred outcomes for residents and providers.

What you bring:
5 years' experience post graduate experience in aged care including working with people with dementia.
Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
Ability to conduct clinical assessments using a range of assessment tools.
Strong organisational skills and the ability to work independently and as part of a team.
A genuine desire to improve the lives of older Australians.
Your own vehicle as travel will be required within your region.
