The position
As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe, and connected within their own homes and communities.
Based in Bacchus Marsh, this role supports clients across Ballarat and Melbourne's western metropolitan region, requiring regular travel to build meaningful relationships and coordinate personalised services that enhance wellbeing, independence and quality of life.

Bring your care coordination skills, client-focused mindset, and passion for making a difference. You'll work closely with clients and their support networks to navigate services, coordinate supports, advocate for individual needs, and help clients achieve their personal goals.
What's required
Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor degree or above)
Experience in case management, care coordination or client services within aged care, community services or a similar environment
Experience completing assessments, developing care plans and supporting clients with complex needs
Ability to manage consumer budgets and support services effectively
Sound computer skills, including Microsoft Office and client management systems
Excellent organisational skills with the ability to prioritise and work autonomously
Understanding of community aged care services, referral pathways and person-centred care principles
Knowledge of aged care legislation, quality standards and sector reforms
Current Victorian Driver's Licence andcar registration
Willingness to travel across Metropolitan & Regional areas
National Police Check
