Care Partner - Transition Care Program (TCP) | Part Time Ongoing | 0.6 EFT (45.6 hours per fortnight | 3 days per week)
About the Role
Are you passionate about supporting older people to maintain their independence, achieve their goals and make decisions that matter to them?
As a Care Partner within the Transition Care Program (TCP), you will play a key role in supporting older people as they transition from hospital to home or ongoing care. Working across both our Home-Based and Bed-Based TCP services, you will provide person-centred support that empowers individuals to navigate their care journey with confidence.
You will build meaningful relationships with clients, families and carers, acting as a trusted advocate and navigator to ensure each person's goals, preferences and aspirations remain at the centre of their care. Working collaboratively with geriatricians and the broader interdisciplinary team, you will help deliver safe, coordinated and personalised care experiences.
This role focuses on promoting:
Choice, control and shared decision-making
Independence, reablement and achievement of personal goals
Dignity, respect and culturally responsive care
Individualised support tailored to each person's needs
Safe and seamless transitions across healthcare settings
Based predominantly in the
Dandenong region
, you will work across both the
Home-Based and Bed-Based TCP programs
,



providing adaptable and responsive support that recognises the unique circumstances of each person.
About Us
The Transition Care Program (TCP) supports older people as they move from hospital to their next stage of recovery, reablementand care.
A policy initiative between theState and Commonwealth Governments, TCP provides short-term, goal-directed support designed to help older people:
Maximise their independence and functional ability
Build confidence following illness or hospitalisation
Remain living safely at home where possible
Make informed decisions about their future care needs
Avoid unnecessary hospital presentations and residential aged care admissions
Through collaborative care planning, low-intensity therapy, personal support and care coordination, we work alongside older people and their families to achieve the outcomes that matter most to them.
About You
You are passionate about supporting older people to live well and delivering person-centred, rights-based care.
You bring:
Advanced skills in care coordination, care planning, dischargeplanningand service navigation
Experience supporting older people through complex healthcare journeys and transitions of care
A strong understanding of person-centred practice and collaborative goal setting
Excellent communication and relationship-building skills
The ability to work autonomously while thriving within an interdisciplinary team
Experience in acute, rehabilitation, community, agedcareor Transition Care settings
A commitment to continuous improvement, qualitycareand positive consumer outcomes
Public healthcare experience (highly desirable)
Mandatory Requirements
Relevant tertiary qualification
Current professional registration (where applicable)
Relevant clinical experience
Current Victorian Driver's Licence
