Bayside Health is seeking an AOD Liaison Outreach Worker (ADLOW) to improve access to AOD services at Southcity Clinic in Melbourne. The role links consumers with high intensity AOD support to suitable community services and works with complex cases to ensure a seamless transition with clear goals.
Part time (0.8 EFT / 64 hours) until January ****, based at St Kilda Road Clinic, you will join an experienced multidisciplinary team and contribute to coordinated care for clients with complex needs.
