What you will do:
Manage a complex Adult Social Care caseload in an effective and person-centred manner.
Undertake comprehensive Care Act assessments, reviews, and support planning.
Complete Mental Capacity Assessments and facilitate Best Interests decision-making processes.
Lead safeguarding enquiries, assess risks, and implement appropriate protective interventions.
Prepare detailed reports, assessments, and Court of Protection documentation where required.
Work collaboratively with health professionals, care providers, and multi-agency partners to coordinate effective support and interventions.
Monitor, review, and evaluate care packages to ensure desired outcomes are achieved and resources are used effectively.
Maintain accurate, timely, and legally compliant case records in line with legislation, policies, and professional standards.
Participate in duty functions and contribute to the ongoing development and improvement of the Adult Social Care service.
Promote independence, wellbeing, and strengths-based practice in all aspects of your work.
