You will coordinate individualized plans, work within a rights-based, person-centred framework, and collaborate with participants, supporters, and clinical partners to deliver culturally protected services. You will manage budgets, documentation and reviews, ensure alignment with program rules, and empower informal carers while navigating aged care pathways and community
