Description

Laynhapuy Homelands Aboriginal Corporation (Laynha) is looking for a caring and reliable Aged Care and NDIS Support Worker to join our Health team. This role provides practical and personal support to Aged Care and NDIS clients living across the Laynhapuy Homelands. You will help clients live safely, independently and with dignity in their own homes and communities.

About Us For more than 40 years, Laynha has supported Yolŋu people living across approximately 30 homelands in East Arnhem Land. Our Health Services team delivers primary health care across the Homelands, working alongside Community Health Workers and other health professionals to support the health and wellbeing of Yolŋu on Country.

What you’ll do

- Travel to Laynhapuy Homelands to provide support to Aged Care and NDIS clients.
- Provide assistance in clients’ homes, including cleaning, gardening, laundry, shopping and transport to appointments.
- Support clients to participate in social activities and stay connected with their community.
- Provide personal care where required and in line with the client’s care plan and your scope of practice.
- Respect each client’s privacy,



dignity, culture and home environment.
- Follow individual care plans and provide services in a safe and culturally appropriate way.
- Complete basic records and documentation about services provided.
- Report any concerns about a client’s wellbeing, changes in their condition or safety hazards to the appropriate staff.
- Work closely with the Health and Clinical teams to support continuity of care.
- Maintain confidentiality and treat clients, families, colleagues and community members with respect.

About you

You’ll need

- Certificate II or III in Individual Support, Community Services or similar, or relevant experience.
- NDIS Worker Screening Check.
- Current manual Australian Driver’s Licence.
- First Aid and CPR certificate.
- NT Ochre Card and National Police Check (or ability to obtain)
- The ability to travel by 4WD and light aircraft and undertake overnight trips in remote locations.
