We have an exciting opportunity for experienced and motivated Care Partner to join our Home Care Services team on a temporary 3 month full time basis. The role will be both providing care coordination and in home Registered Nursing services to our community clients. You will be working to provide a high quality service of managing and maintaining a growing client base by assisting each person individually to attain their goals and maximise their independence and abilities to meet their everyday needs.

Your responsibilities may include:

- Deliver high‑quality, person‑centred Registered Nursing clinical care to clients in their own homes across Sydney’s South Eastern suburbs, supporting complex care needs, chronic disease management, and protected, dignified ageing in place.
- Collaborative Planning:Partner with participants to develop an individualised care planbefore or on the day services commence, drawing from the participant’s assessed needs, goals, and preferences identified in their Mac Support Plan and Notice of Decision.
- Ongoing Support and Planning:



Be responsive to changes in client’s need through on-going assessment and planning;
- Risk Management:Conduct home and personal risk assessments, identifying mitigation strategies while respecting the participant’s dignity of risk.
- Wellness and Reablement:Apply holistic approaches that focus on "doing with" rather than "doing for," helping participants regain or maintain functional and cognitive capabilities.
- Advance Care Planning:Support participants in completing and reviewing advance care planning documents as required.
- Work within clinical governance framework and ensure that complex clients care planning has regular and appropriate clinical oversight.

Essential Criteria:

- University-level qualification in a relevant health-related discipline such as Bachelor of Nursing.
- Have experience in Aged Care or a clinical background.
- Be committed to Health Care, St Luke’s Values and St Luke’s Code Of Conduct;
- Be committed to ongoing educational and professional development;
- Be an energetic and enthusiastic team player and contributor;
- Successfully manage client case load, their budgets and their roster needs.
- Well-developed communication and interpersonal skills, particularly building healthy rapport with clients, carers and colleagues;




- Be proficient in office computer systems and be willing to share knowledge where necessary;
- Maintain a current driver’s licence with access to a car with comprehensive insurance;
