Eastern Health's Community Mental Health Transition to Specialty Practice (TSP) Program, commencing in February 2027, offers a supported pathway for experienced acute mental health nurses to transition into community-based care through structured learning, mentorship and hands-on clinical experience.
n

- Continuing Care Units (CCU) - Box Hill and Maroondah
n
- Maroondah Early Psychosis Team
n
- Infant, Child and Youth Mental Health Service (ICYMHS) Access
n
- Older Adult Community Mental Health
n
n
*Please note: MST positions require a minimum of 18 months' experience in contemporary acute mental health settings.

About the Program
n
This 12-month Community Mental Health Transition to Specialty Practice Program provides a structured and supportive learning pathway for nurses moving from acute inpatient mental health settings into community practice.
n
The program includes:

- Continuity in one clinical area for 12-months
n
- Progressive caseload
n
- Dedicated education and professional development sessions
n
- Group clinical supervision
n
- Mentoring and coaching
n
- Structured support from experienced clinicians and leaders

About You
n
You are a motivated, compassionate and recovery-focused Registered Nurse currently working in acute mental health who is ready to take a step into community mental health nursing.
n
You are passionate about supporting people in their recovery journey and excited by the opportunity to build your confidence, autonomy and specialist community mental health skills through a structured learning program.
n
We are looking for nurses who:
n
n
- Build strong therapeutic relationships with consumers, carers, families and multidisciplinary teams.
n
- Deliver safe, person-centred mental health care, including assessment, care planning, intervention and evaluation.
n
- Respond effectively to risk, deterioration and complex clinical presentations using sound clinical judgement.
n
- Communicate confidently and coordinate care across health services and supports.
n
- Demonstrate resilience, adaptability and a commitment to reflective practice, feedback and continuous learning.
n
- Contribute to a culture of safety, professionalism, compassion and recovery-focused care.
n
- Bachelor of Nursing or other recognised equivalent qualification leading to condition-free registration in Australia.
n
- Current registration as an RN with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA.
n
- Minimum 12 months post-registration nursing experience, including at least six months in contemporary acute mental health settings (18 months required for MST positions).
n
- Ability to work a rotating roster for some areas.
n
- Current Victorian Driver Licence.
n
n
Highly Desirable
n
n
- Post Graduate qualification (or higher) in mental health nursing or working towards
n
- Experience in an acute mental health setting at a public hospital
n
n
