Disability Support Worker / Enrolled Nurse – Female
About the Position
We are seeking an experienced female Disability Support Worker who is also a currently AHPRA-registered Enrolled Nurse (EN) to provide 1:1 in-home and community access support to female participant in the Wynnum area.
The participant receives 24/7 in-home support, and this position is primarily a disability support role with additional participant-specific nursing and clinical responsibilities.
We are looking for someone who understands that quality, person-centered support is about more than completing daily tasks. The right person will support the participant to maintain independence, build capacity, exercise choice and control, and work towards their individual goals.
Alongside day-to-day disability support, your EN qualification and clinical experience will provide an additional level of clinical knowledge and oversight in relation to the participant’s health needs, monitoring and nursing-related supports.
Key Responsibilities
- Provide medication support and participant-specific nursing supports within scope of practice.
- Monitor the participant’s health and wellbeing, including relevant health observations, diabetes management and wound care, and appropriately escalate any changes or concerns.
- Provide person-centered 1:1 support with personal care, daily living, household tasks and community access.
- Promote independence, capacity building, choice and control while supporting the participant to achieve their individual goals.
- Maintain accurate and timely progress notes, medication records, health observations and other participant documentation.
- Working collaboratively with the participant, support team, management and relevant health professionals to maintain protected, consistent and high-quality supports.
Essential Requirements
Applicants must have
- Current registration with AHPRA as an Enrolled Nurse (EN).
- Minimum Certificate III in Individual Support, Disability or an equivalent relevant qualification.
- Demonstrated experience providing disability support within an in-home and/or community setting.
- Experience with BGL monitoring, diabetes management, temperature, blood pressure and pulse observations.
- Experience with medication administration and management.
- Wound care knowledge and experience.
- Strong understanding of person-centred practice, capacity building, choice and control.
- Ability to recognise and appropriately escalate changes in a participant’s health or wellbeing.
- Ability to complete accurate and timely participant documentation, including progress notes, medication records and health monitoring documentation.
- Strong communication skills and the ability to work effectively as part of a support team and alongside relevant health professionals.
Availability Requirements
- This position forms part of a 24/7 in-home support roster.
- Applicants must have flexibility to work a combination of weekdays, weekends, public holidays, sleepover shifts (inactive sleepover 10pm-6am) and community access supports incorporated within AM/PM support allocation.
- Regular and consistent availability will be highly regarded.
