About the Role:

Are you passionate about providing compassionate patient care and making a difference in people's lives? Echuca Regional Health is seeking a dedicated Health Care Worker to join our Subacute team and support patients in achieving the best possible health outcomes.

Health Care Workers work under the supervision and direction of Registered Nurses, assisting with delegated aspects of patient care as part of a collaborative multidisciplinary team. This role provides hands on support to patients while ensuring dignity, privacy, safety, and person-centred care are maintained at all times.

Key responsibilities include:

· Assist patients with activities of daily living, including personal hygiene, grooming, feeding, hydration, mobility, transfers, positioning, and elimination needs.

· Support patients through social and diversional activities.

· Undertake delegated observations such as vital signs and blood glucose monitoring.

· Provide constant patient observation when required.

· Maintain a safe and supportive patient environment.

· Communicate effectively with patients,



families, carers, and members of the multidisciplinary team.

· Recognise, document, and report patient care concerns promptly to the supervising Registered Nurse.

· Adhere to infection prevention and control standards and organisational policies

Qualifications/experience required:

· Certificate III in Health Services Assistance, Certificate III or IV in Aged Care or Individual Support, or equivalent qualification.

· Sound knowledge and skills in patient care.

· A commitment to patient-centred care.
