About the role
n
As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client's home or residential accommodation.

Key Responsibilities
n
n
- Support and education for clients with terminal diagnoses
n
- Education and support for families and carers
n
- Administration and preparation of s/c medications and CSCI for symptom management
n
- General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
n
- Accurate/ Timely documentation in all client records.
n
- Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
n
- Complete initial assessments/develop care plans to ensure best outcome for our clients.
n
- Monitor and update care plans to ensure appropriate care is provided
n
- Recognise and respond to clinical deterioration, with appropriate escalation.
n
- Promoting best practice and initiating quality improvement activities
n
n
What you bring
n
n
- Current AHPRA Nursing
n
- Minimum 2 years palliative care nursing experience, community experience desirable
n
- Post graduate qualifications (or working towards) in Palliative Care
n
- NDIS Clearance (or willingness to obtain one)
n
- Police Clearance with in 6 months (or willingness to obtain one)
n
- Use of own car - KM allowance provided
n
- Full drivers licence, reliable and comprehensively insured car
n
- CPR and Manual handling
n
- A desire to join a team committed to the care of others in a mission-based organisation
