? Why This Role Is Different
This isn’t a high-volume, transactional support coordination role. At ICAH, our Level 3 Support Coordinators work with some of Australia’s most clinically complex NDIS participants — people who are ventilator-dependent, tracheostomy-managed, or living with progressive neuromuscular conditions. You will work at the intersection of clinical care, NDIS funding navigation, and genuine family advocacy. Your decisions and advocacy change lives.
The Role
Reporting to the Clinical Operations Manager, you will lead Specialist Support Coordination (Level 3) for a caseload of high-complexity NDIS participants, coordinating the full ecosystem of supports needed to keep medically dependent people safely at home and out of hospital.
Key Responsibilities
- Manage a specialist caseload of NDIS participants with complex, high-intensity support needs including ventilator dependence, tracheostomy care, and progressive neurological conditions
- Develop, implement, and review highly individualised NDIS plans, identifying appropriate funded supports and navigating complex plan management
- Coordinate hospital-to-home discharge pathways in collaboration with ICU teams, discharge planners, and ICAH clinical nursing staff
- Advocate powerfully on behalf of participants and their families within the NDIS system, resolving barriers and escalating where needed
- Build and maintain relationships with LACs, plan managers, complex plsn managers,
allied health providers, and hospital discharge teams
- Prepare high-quality NDIS plan review submissions, progress reports, and support budgets
- Participate in internal case conferences and contribute to a culture of clinical excellence and family-centred care
- Maintain accurate and compliant records in line with NDIS Practice Standards and ICAH’s ISO 9001:2015 quality framework
- Travel within metropolitan Melbourne and regional Victoria as required; occasional interstate travel may apply
What We’re Looking For
We welcome applications from candidates with a background in allied health, nursing, social work, or specialist support coordination — what matters most is your depth of experience with complex, high-intensity NDIS participants and your ability to advocate fiercely for people who depend on you.
Essential
- Demonstrated experience as an NDIS Level 3 Specialist Support Coordinator
- Proven track record coordinating supports for high-complexity or high-intensity NDIS participants with 24/7 funded nursing care
- Deep understanding of the NDIS Practice Standards, Pricing Arrangements, and plan review processes
- Experience navigating hospital discharge pathways and working with clinical multidisciplinary teams
- Exceptional written and verbal communication, with the ability to draft compelling plan review submissions and reports
- Highly organised, self-directed, and able to manage a demanding caseload with competing priorities
- Empathetic, resilient, and committed to the rights and dignity of people with disability
- Current driver’s licence and willingness to travel across metropolitan Melbourne, regional Victoria, and occasionally interstate
- NDIS Worker Screening Check (or willingness to obtain)
- Working with Children Check (or willingness to obtain)
Highly Desirable
- Proven track record as level 2 or level 3 NDIS support coordination or Clinical background in nursing, occupational therapy, physiotherapy, speech pathology, or social work preferred
- Experience with medically complex, ventilator-dependent participants and/or those requiring tracheostomy management, home TPN, IV fluids management
- Familiarity with NDIS Supported Independent Living (SIL), SDA, or in-home intensive care funding streams
- Existing relationships with hospital discharge teams, ICU units, or rehabilitation facilities
