We are seeking a agile, experienced support worker to assist with our Sustaining Tenancy program. This role is primarily focused on client outreach support
Part time fixed term role till the 30/06/2027
Working 30 hours/ 4 days per week from 8:30am to 4:30pm
The role of the support worker is to be responsible for supporting the provision of quality, flexible, individualised client directed support services for people with mental health issues and disabilities; are over 65 or other complex needs living within the community.
Communify is a multi-service organisation that provides a range of programs and activities for families and individuals in the community to enhance their quality of life.

Qualifications and Experience-
Desirable
- Experience working directly with a broad range of people including mental health, disabilities living in the community
- Experience and knowledge around strength based, recovery oriented support
- Experience in capacity building skills
- Ability to work independently and in a team
- Sustaining Tenancies Team work closely alongside Aboriginal and Torres Strait Islander peoples, while this is not an identified position, we strongly encourage First Nations Women to apply.
Required
- Professional experience working with people experiencing mental health issues and/or at risk of losing their housing
- A current Open QLD driver’s license, Blue Card and ability to undergo a National Police Check
