About the role: * Practise autonomously to deliver advanced, holistic and culturally safe clinical care within the Woorabinda rural and remote Indigenous community. * Lead and coordinate Primary Health Care activities across key program areas including Chronic Disease, Women and Child Health, Community and Public Health, or the Community Home Support Program. * Provide clinical leadership and support to Registered Nurses, Enrolled Nurses, Indigenous Health Workers and other team members, including guidance in complex clinical decision-making. * Develop, implement and evaluate health programs, clinical practices and protocols, ensuring evidence-based, high-quality care and continuous improvement. * Work collaboratively with the community, multidisciplinary teams and external agencies to strengthen community health and wellbeing while promoting culturally respectful care for Aboriginal and Torres Strait Islander Peoples. About you: * Current registration as a Registered Nurse with AHPRA; additional qualifications or learning in Community and Primary Health Care, Diabetes Education, Rural and Remote Nursing or Immunisation highly regarded.
* Advanced clinical knowledge and skills in Primary Health Care, including screening, assessment, care planning, coordination and delivery of quality health programs. * Ability to provide clinical supervision, guidance and support to regulated and unregulated care providers, including orientation, induction and ongoing professional development. * Strong communication and interpersonal skills, with the ability to collaborate effectively with healthcare teams, patients, families, community members and external agencies.
