About the role The Registered Nurse (RN) plays a critical role in delivering direct patient care in a Community industry. RNs are responsible for assessing patients' conditions, implementing nursing care plans, administering medications, and collaborating with other healthcare professionals to ensure optimal patient outcomes. This role demands a combination of clinical knowledge, compassionate patient care, and effective communication skills.

Key responsibilities

- Conduct thorough physical assessments of Clients
- Monitor and document vital signs, symptoms, and changes in Clients conditions
- Evaluate clinical data
- Visit community and SIL houses on a regular basis
- Develop, implement, and evaluate individualized care plans
- Collaborate with physicians, other healthcare providers, and multidisciplinary teams
- Educate patients and families about health conditions, treatment options, and preventive measures
- Administer prescribed medications and treatments per physicians' orders
- Monitor patients for adverse reactions or side effects
- Serve as the primary advocate for patient needs, concerns, and rights

About you





- A valid Bachelor's degree in Nursing or Associate Degree in Nursing (ADN)
- Must hold a valid and active Registered Nurse (RN) license in the state or region of employment
- Previous experience in Community nursing
- Robust clinical assessment skills
- Excellent communication and interpersonal skills
- Ability to work effectively under pressure in fast-paced environments
- Knowledge of medical equipment, clinical procedures, and patient care technologies
- Compassionate, empathetic, and patient-centered approach
