Moving from hospital back into everyday life can be a significant transition. The right support can help make that next step feel more manageable, connected and achievable.

We’re looking for a Recovery Support Worker to join our Hospital to Home team in Caboolture on a 9-month fixed-term, full-time contract. You’ll work alongside people as they transition from acute mental health care back into the community, providing practical, person-centred psychosocial support that builds confidence, independence and connection.

Your work will be shaped around what matters to each person. You might support someone to prepare for discharge, reconnect with everyday routines, build practical skills, strengthen community connections or work towards goals identified in their Personal Recovery Plan.

This is a role where relationships matter. You’ll provide consistent support across hospital and community settings, work closely with services and stakeholders, and help people feel supported as they take the next steps in their recovery journey.

Your Key Responsibilities

- Support clients through hospital discharge planning and their transition back into the community.
- Visit clients in hospital and provide ongoing psychosocial support following discharge.
- Work alongside clients to identify and progress goals within their Personal Recovery Plans.
- Support everyday living skills, capacity building, independence and community participation.
- Help clients strengthen social and community connections that support their wellbeing.




- Build effective working relationships with internal teams, health services and other community providers.
- Manage an allocated caseload, documentation and required outcomes with the support of regular supervision and your broader team.

You Will Bring

You’ll bring a genuine interest in working alongside people experiencing mental health challenges, together with the ability to build respectful relationships and provide practical, recovery-oriented support.

You will also have:

- A Diploma in a human services related field and/or demonstrated relevant experience working with people experiencing mental health challenges.
- The ability to work independently while contributing positively within a collaborative team environment.
- Strong organisation and time management skills, including the ability to manage a caseload, documentation and required service outcomes.
- Excellent communication and interpersonal skills, with the ability to engage effectively with clients, colleagues, stakeholders and service providers.
- Sound skilled judgement, boundaries and integrity.
- Willingness and ability to meet applicable occupational assessment,



screening and vaccination requirements relating to specified infectious diseases.
